I like how the response is always a bunch of bullshit statements, like "safety is our top priority", and "we will get to the bottom of this". The sign was not properly bolted down, that's the only explanation. Whatever checks and inspections they say they did, they didn't do. They can say whatever they want about what their "priorities" are, but it doesn't matter. Someone didn't do their job, and they clearly don't have a system in place that guarantees safety.

The problem is that in Australia, the bolts tighten the opposite direction.
So it's "lefty - toighty, roighty - loosey."
It's very confusing.
