I was watching something a while back talking about elevator accidents. More than you would care to think about. Some are defective elevators breaking at inopportune times. Others are human error. One lady got sliced in half as the elevator went up several floors while door was open (I think something similar happened here). In that case they had upgraded the elevator software and the technician left the elevator computer in "testing mode" (this was required to upgrade the software) when it was put back into service. This turned off the protocols that prevented the cars from moving while the doors are open. After watching this special I am always quick to enter or exit an elevator. Also take the stairs down where possible.
